Practice run before you claim for real

The online claim form asks for the same handful of answers every single time. Below is a practice copy in the same field order, with the fixed ones ready to copy. Work down it once here, then do it for real in five minutes.

Your airline’s claim portal

Sign in to your own staff hub first — these go straight to a login screen otherwise. Each airline arranges it differently, but the claim form itself asks for the same handful of answers, which is what the rest of this page rehearses.

CX · Cathay Pacific ↗ UO · HK Express GBA · Greater Bay Airlines HKA · Hong Kong Airlines Air Hong Kong

Cathay: open the hub, follow the link above, pick your role — then Medical Plan Portal takes you into the claim system. The screen-by-screen version, plus how to read your remaining balance, is on the Cathay guide.

The greyed-out ones are not wired up yet. We have not published a direct link for HK Express, Greater Bay, Hong Kong Airlines or Air Hong Kong. Reach your medical portal through your own staff intranet in the meantime — everything else on this page applies to you just the same.
This page submits nothing, anywhere. There is no login here and no connection to your insurer. Anything you type below stays in your own browser and disappears when you close the tab — we never see it. Use it to rehearse, then copy your answers across.

The answers that never change

Same every visit. Tap to copy, paste straight into the real form. The first block is a physiotherapy session at CPR Physio; the second is manual therapy with Aviator Wellness under your CMT benefit.

Claiming manual therapy instead? Aviator Wellness is registered in the CMT programme under Manual Therapy, and that is a different lane with a different pot — use the three values below instead of Local Outpatient / OP - Physiotherapy. How the CMT benefit works →

Complementary Therapy CMT
CMT — treatment
Manual Therapy
CMT — provider / practitioner name

About the amount. The price is the one thing here that is not fixed, so it is not listed above — enter the figure printed on the receipt you were given for that visit, exactly as it appears. Your receipt also records how the fee was settled, including the part covered by earned Foundation credits, so nothing needs explaining away. If your administrator asks about it, answer plainly. Keep the receipt for at least six months; administrators can request the original well after the fact.

The practice run

Same order as the real form. Fixed answers have a Copy button; the rest are yours to fill in.

First, know which lane you are in

Two different treatments, two different pots. The form is the same shape either way — only these answers change.

Lane 1 — Physiotherapy

  • Treated at CPR Physio
  • Claim type: Local Outpatient
  • Treatment: OP - Physiotherapy
  • Provider: Jake’s Physiotherapy Centre Company Limited
  • Comes out of your outpatient pot

Lane 2 — CMT

  • Treated at Aviator Wellness, by Aviator Group
  • Claim type: Complementary Therapy CMT
  • Treatment: Manual Therapy
  • Provider on the form: Aviator Group Limited
  • Comes out of your separate CMT limit

Because CMT usually holds a limit of its own, a manual therapy session does not eat into the pot your physiotherapy comes from. How the CMT benefit works →

1

Patient name

A dropdown of you and your covered spouse. Pick whoever was treated.

2

Was the treatment due to duty?

Yes / No. Answer honestly — a duty-related injury may be handled under a different benefit.

3

Was it an injury?

Yes / No.

4

Do you need a Certified True Copy?

Normally No for an outpatient physiotherapy session. Answering Yes means you must post your original documents to the claims administrator, typically within 90 days.

5

Claim type

Choose Local Outpatient for a physiotherapy session at CPR Physio. Choose Complementary Therapy CMT for manual therapy with Aviator Wellness — different treatment, different pot.

6

Country / region, and currency

Hong Kong, and HKD.

7

Date of service

The treatment date on your receipt — not today’s date. Use the form’s own date picker rather than typing it, so the format always matches.

8

Provider name

Either of our registered names works — copy one from the block above. Your physiotherapist’s own name is also accepted.

9

Diagnosis

A dropdown plus a free-text box. Use whatever is written on your referral letter. We deliberately do not suggest one here — the diagnosis is your referring doctor’s call, not ours.

10

Treatment, and price

Pick OP - Physiotherapy — or Manual Therapy if this was an Aviator Group CMT session — then enter the amount shown on that visit’s receipt. The price box takes digits only: no dollar sign, no comma.

11

Medication rows

Leave blank. A physiotherapy session has no medication to declare.

12

Contact number

Country code 852, area code blank, then your own mobile.

13

Upload your documents

Your receipt and your referral letter. Up to five files, each under 4 MB, PDF or JPG only. More than five pages? Combine them first — the form will not take a sixth file.

14

Comment

Optional. Most physiotherapy claims need nothing here.

15

Read the four declarations, then tick

Read them properly rather than clicking through. They cover what you paid, keeping your originals for six months, whether any part of the claim has already gone to another party, and the consequences of misrepresentation. If anything about your situation does not match a declaration, stop and ask your claims administrator before you submit.

The trick that halves your next claim

←
Once your claim has gone through, press your browser’s Back button. The form comes back with everything you typed still in it. For your next session you only change the date — provider, claim type, treatment, price and phone are already sitting there. A five-minute job becomes a thirty-second one.
Two rules if you use it. Only go back after you have seen the confirmation and have your OCN written down — going back too early can lose the submission. And re-read every field before you submit again, especially the date and the price: a stale date is the fastest way to have a claim bounced, or to file the same one twice by accident.

Sign in to your staff hub first, or the link will bounce you to a login screen.
